CohesiveCare Medical is seeking a NP/PA to join our dynamic mobile Wound Care Services team. Roles and Responsibilities
- Evaluate patients in person and assess patients’ unique needs, creating treatment care plans. Select, order, and perform tests and procedures to best treat the patient. Provide wound care and treatment to patients with acute or chronic wounds.
- Perform wound care procedures as deemed necessary and appropriate for the plan of care. Define and document patient diagnoses.
- Educate patients and families on best practices to promote wound healing.
- Debridement of wounds by various methods completed safely during the patient visit. Application of topical wound care and other wound management products.
- Document patient assessments, interventions, and outcomes accurately and timely
- Collaborate with a supervising physician to ensure a smooth recovery process.
- Function as a case manager for wound care patients. Identify care needs, evaluate support systems, and identify patient teaching needs during the wound healing process. Coordinate office visits, pre-procedure work-ups, and patient education with the multidisciplinary team. Coordinate the use of clinic staff, exam room space, and supplies. Foster continuity of care between our team and caretaker (facility, home, hospital, home health agency, etc).

- Strong In-depth knowledge of wound care techniques, products, and equipment.Familiarity with discharge planning and coordination of care

- Strong interpersonal skills. Have a kind, supportive, and empathetic demeanor dealing with patients and their family members.
- Strong attention to detail. Write detailed notes in patient charts, and create treatment plans with strong attention to detail.
- Strong time management skills. Be able to schedule patients and administer to patient needs in a timely manner administering treatment accurately.
- Strong collaboration skills. Be able to work with patients, family, members, medical staff, and physicians in following treatment plans and physician orders effectively. Familiarity with discharge planning and coordination of care
Qualifications and Education Requirements
- Current and active license as NP or PA
- Completed an accredited medical program and licensed in the state of California.
- 1+ Years of Wound Care Experience required. This includes providing basic wound care in hospital, skilled nursing, clinic and/or home health settings or any combination.
- Wound Nurse Certification is optional (WOCN, CWS, or WCC).
- Current BLS certification for HealthCare Providers.
- Previous experience in a hospital, home health, skilled nursing facility, assisted living facility or clinic preferred.
- Understand HIPAA and OSHA regulations.
- Proficient in EHR medical charting system. Tebra & PointClickCare experience a plus.
- Proficient in Google Apps like Docs, Sheets, Slides and Microsoft Office.
- Valid Driver’s License and Automobile Insurance.
- Bilingual in Spanish is a plus.Hourly
Hourly Pay $55-$65
